The Seniors Blue Book of Salt Lake City is published semi-annually in March and September. The guides are
distributed to over 200 locations convenient for seniors and senior professionals such as hospitals;
libraries, senior centers, churches, government agencies and special senior events. You can also find the
guides in local Smith's Pharmacies.

The State of Utah provides a WAIVER for individuals age 65 and older to provide services statewide to help
older adults remain in their homes or other community based settings. With this program individuals are able

This program is designed to help people transition our of a nursing facility and re-establish themselves
