True to our tradition of providing cutting-edge health care services, South Davis Community Hospital became one of the first long term acute care (LTAC) hospitals in the state of Utah. Our patients typically have an unstable illness or injury, often combined with a complicating condition. Many are technology-dependent, requiring equipment such as ventilators. We provide skilled care for patients with conditions that are not expected to resolve quickly. Unlike traditional hospitals that are required to move patients through their systems quickly, at SDCH our care is focused on a longer-term course of treatment. This shift in emphasis provides our patients with key benefits such as: facility and staff that are designed to meet the privacy, comfort, and other needs of long-term patients and cost-effective care that avoids the high price of a lengthened stay at traditional hospitals.

Individuals that have skilled needs to the extent that the services they need cannot be found through home care, residential care or assisted living facilities, qualify for skilled nursing care. Our skilled nursing care offers both short and long term care options for those with serious problems that require 24-hour nursing care. Our goal for all patients and residents is that they enjoy life at the highest level possible, and achieve their maximum physical and mental independence.

Our experienced staff will assist in dealing with Medicare/Medicaid Agencies and private insurance companies to maximize patient benefits. Cost-effective treatment programs and planning will enable significant extensions of health insurance benefits and periods of coverage, resulting in more time for rehabilitation and skilled nursing care.

Upon admittance, an initial assessment is completed for every patient. The hospital's multi-disciplinary treatment team initiates a care plan with specific goals. Patients and family members are encouraged to participate in the development and updates of these individualized care plans.

South Davis Hospice is a program of care which meets the physical, emotional, spiritual, and social needs of people in the last stage of life's journey. Hospice recognizes dying as a normal process, neither hastening nor postponing death. Hospice services focus on the patient and family as the unit of care. It is a unique care concept to help terminally ill patients live life as fully and as comfortably as possible, and to help loved ones cope after death.

Given the choice, most of us would prefer to be at home when we are sick. This is particularly true for those who are terminally ill. However, the family may be unsure of their ability to provide appropriate home care for their loved one. South Davis Hospice offers a coordinated system of physical, emotional, and spiritual care that is tailored to fit each patient. Everything we do at South Davis Hospice serves to help the patient and family live as comfortably as possible and to enable them to draw closer to each other.

Patients and families who choose hospice form the core of the hospice team and are at the center of all decision making.
