We can provide care services in a home, senior living community or medical care facility

ComForcare Southeast Valley Phoenix provides personal care and companionship services for individuals in their homes, senior living community or medical care facility. Research has shown that people stay healthier and recover more quickly from illness or surgery in the comfort of their own home. With today's busy lifestyles, family and friends often do not have the time and energy required to keep a loved one safely at home or at times support them through a stay at a care facility. ComForcare Southeast Valley Phoenix can provide the necessary help to meet all of the daily needs of the homebound individual whether they be in their own home, senior living community or medical care facility.

We match caregivers to you and your family based on the skills needed and personality traits necessary for a good relationship. Our qualified and trustworthy caregivers put families and clients at ease. For family members we can relieve some of the pressure of consistently providing for a loved one. We offer 24-hour service, 7 days a week including holidays. All staff is bonded, insured and caregivers are supervised on site.

We can provide Personal Care Services such as assistance in the bathroom, dressing and oral hygiene, recording vital medical signs, hands-on bathing and transfers. We also offer Companionship Services such as meal preparation, medication reminders, housekeeping, driving to doctor appointments and grocery shopping.

Chandler, Gilbert, Mesa Tempe, Apache Junction, Sun Lakes Queen Creek, Higley

In Pinal County we provide services to:
Queen Creek Florence, Coolidge, Maricopa, Arizona City, Oracle and Casa Grande

When you first call us a ComForcare Southeast Valley Phoenix scheduler will make an appointment at the potential client's home with our RN or Social Worker to assess the case. The client's health and activities of daily living (ADL's) are reviewed to determine the appropriate care plan. If the client's needs require a nursing home, we will make the appropriate referral. In most instances, a written care plan is developed that outlines specific services appropriate for the new client. The care plan is then reviewed with the client, legal guardians or other family members and service agreements are signed for immediate or future service.

ComForcare's schedulers will then match a caregiver to a client based on skills needed for care, personality and availability. Caregivers are oriented to the client with detailed explanations of special needs and requirements.

Our schedulers routinely call caregivers and clients to monitor and review care, client condition and provide additional assistance. Regular visits are made to client's homes to also assess the client and caregiver relationship. Our caregivers also are required to record daily documentation of client's care and relay special circumstances to the ComForcare Southeast Valley Phoenix for discussion with the client, legal guardians or other family members.

All of our caregivers have completed very detailed screening and background checks. Every caregiver is bonded and insured. Further, the majority of our caregivers have advanced training and they have been oriented to our system of home health care and also oriented to the needs of our clients. ComForcare Southeast Valley Phoenix also will work with insurance companies for coverage and accepts private pay.
