Jefferson Parish - Public Hurricane Evacuation Assistance

The Jefferson Council on Aging is currently working with the Jefferson Parish Office of Emergency Management
in assessing the need of seniors and persons with disabilities with regard to Hurricane Evacuation
Assistance. JCOA's Aging and Disability Resource Center is accepting applications for those citizens who are
elderly and handicapped and do not have the means to evacuate themselves. Please pick up an application
packet from Jefferson Council on Aging. Applicants who are medically fragile or technology dependent will
need to have their primary care physician complete the medical form included in the packet in order to
expedite triage at the time of evacuation. The packet must be completed in its entirety and returned to the
Jefferson Council on Aging to be placed on the database list to be given to emergency personnel at the time
of a mandatory evacuation.

Louisiana SenioRx
LouisianaSenioRx.org is a program that connects qualified, low-income people with discount prescription
drugs, direct from the pharmaceutical manufacturer. The program was created in 2003 by the Louisiana
Legislature to provide resources to help senior citizens access and apply for manufacturer's discount cards
and pharmaceutical assistance programs. The program is administered by the Governor's Office of Elderly
Affairs.
