Contin-U-Care Outreach Services, Inc. is a Social Service Agency in the state of Louisiana that takes great pride and dedication in assisting elderly and disabled adults. We are a team of professionals committed to delivering the highest quality service to all individual and their family members. We are committed to protecting the individual's choices that will enhance their life based upon their goals. Contin-U-Care Outreach Services is ready to serve, dedicate and prepared to meet all individuals' needs in personal care and respite care services.

Our Outreach Services ...
We provide a flexible of array of outreach services that include individualized and family support, day or night to people who are eligible to enter an institution but choose to remain living in the community. Our services include:

• Personal Care Services - (PCS)
• Supervised Independent Living - (SIL)
  Companion care, day or night in the individual's home in the home in the absence of the family member who is the usual caregiver
• Community Integration Development (CID),
• Daily Living Services include assistance with eating, bathing, dressing, personal hygiene and other activities of daily living
• We can also accompany individuals to clinics, physician offices and other appointments
• As part of our total services we can assist individuals with homemaker and companion services such as housekeeping, shopping and community outings
• We also assist clients with achieving or maintaining personal development

Medicaid Programs ...
Contin-U-Care Outreach Services, Inc. as a Social Service Agency in the state of Louisiana is authorized to provide a variety of programs including Community Integration Development (CID), Elderly and Disabled Adults (PCA) and Supervised Independent Living (SIL). These plans provide for a variety of assistance either in the home, the community or both. There are state qualifications and criteria to be included in these programs. Call us today for an interview and we will review your situation. At this time we do not expect to charge a fee to qualifying clients for these programs. Medicaid will be Contin-U-Care Outreach Services source of payment.
