Home Care Services

FirstChoice HealthCare Services recognizes the importance of tailoring home care services to our patients' needs using a holistic approach. Therefore, each patient has a Case Manager to consult with the physician and an interdisciplinary team when developing a treatment plan for the individual. The use of home care services often makes the difference between placement in a nursing home and independent living. Our Certified Nursing Assistants (CNAs) or Personal Care Providers can assist with activities of daily living such as bathing, dressing, skin care, hair care, catheter care, exercise program, meal prep, blood pressure, light housekeeping, companionship, socialization, meal preparation, transportation for grocery shopping and doctor appointments and many other services. Home care provides greater privacy, convenience, and most importantly, a chance to remain independent in familiar surroundings.

Outpatient & In-Home Therapy

FirstChoice HealthCare Therapists can come to your home under Medicare Part B or you can visit one of our clinic locations located in Golden and Lakewood, Colorado. The FirstChoice HealthCare Outpatient Clinics specialize in Patient Centered Rehabilitation by designing a program specific to your needs. Services include:

Our highly skilled staff is put through a rigorous screening process including criminal and driving background checks as well as reference and credit checks. Our training program includes CPR certification, on-going educational in-services and skills set. All employees have photo ID badges to put your mind at ease when someone walks through your door. • All cases are opened and case managed by a Registered Nurse, giving our patients the benefits of a clinical eye from day one. As Case Manager, the RN acts as an educated clinician to evaluate changes in condition, while also serving as a knowledgeable medical professional who manages patient care and coordinates services. Having a dedicated Case Manager for every patient provides the individual and his/her family with a heightened level of confidence and assuring peace of mind.
• Private Pay - If insurance no longer covers your health care needs, one of our private pay programs may be just what you need.
• Referral Process and On-site Assessment - We are available to assist you with your inquiry 24-hours a day, 7 days per week.
• FirstChoice HealthCare is available to open and staff cases on the weekend
