The Crossroads staff is made up of compassionate people who truly enjoy and understand seniors. Our residents and their families are rest assured they are receiving the most attentive care possible. Caregivers are available twenty-four hours a day seven days a week.

Crossroads Campus in Delta provides assisted living apartments and lifestyle for seniors in and around Delta County. The location is conveniently located within minutes of the hospital.
The spacious 49-bed facility is beautifully decorated to provide a cheery atmosphere for the residents. Currently, there are six floor plans to choose from.
For seniors who do not require daily assistance there are also sixteen two bedroom independent living homes at Aspen Circle. - see below.

Seniors enjoy a graceful atmosphere and assisted living lifestyle at Crossroads at Rifle. This 32-resident facility is designed and decorated inside and out with a western rustic theme. The interior design is reflective of the rural culture in the area. Residents from the surrounding communities appreciate the homey feel of the community and the comprehensive assistance.
Crossroads at Rifle offers five different floor plans to choose from. The community also offers a variety of activities on a daily basis. The Rifle staff provides exemplary one-on-one assistance for all of it's residents. At Crossroads at Rifle Assisted Living residents will find the following services and amenities:
