Using a team approach, the program provides skilled nurses, physical therapy, occupational therapy, medical social work, and home health aides to meet each patient's individual needs.

With all the services provided in the comfort of the patient's home, the Diabetes Self-Management & Education Program offers Intense Therapy Assistance from skilled nurses who will monitor blood sugars and administer insulin up to four times daily;

Vision Assistance for those who are unable to see the lines on the syringe to draw up the correct insulin dosage; Dexterity Assistance for those who are unable to manually insert the needle into the insulin syringe to draw up the correct dosage; and Anxiety Assistance for those who suffer from mental or emotional issues that affect self-administration of insulin injections.

By taking the necessary steps to control Diabetes, patients can benefit from 1st Choice's Diabetes In-Home Self Management Program and reduce the risk and complications associated with the disease. 1st Choice will assist with insurance, pre-authorization, doctor referrals, and scheduling. Additionally, most services are covered 100% by Medicare.
