Under Original Medicare and Medicaid, health services delivery is fragmented. The resources necessary to stay as healthy as possible exist in different places, making it difficult for people with complex health needs to receive the care they need in the appropriate setting. For example, multiple independent physicians often do not have a complete picture of a patient's treatment plans or the full roster of medications a patient may be taking.

Moreover, multiple payment systems (e.g., Medicare, Medicaid, long-term care insurance) cover different health and well-being needs. Finally, Medicare and Medicaid were designed primarily for acute care, not chronic, long-term illnesses. There is no systematic approach to prevention and early identification of changes in condition; and poor coordination of transitions across care settings. These factors often lead to poor health outcomes that are draining for patients and physicians and unnecessarily costly.

It serves more than 200,000 people nationwide through Medicaid, Medicare, and private-pay health plans, programs and services. All Evercare offerings are designed to help people to stay as healthy and independent as possible in the most appropriate setting of their choice.

At the heart of the Evercare approach are Nurse Practitioners and Care Mangers, who become trusted partners in helping develop and manage personalized care plans for members. They coordinate multiple services; help facilitate better communication between physicians, institutions, patients and their families; and help ensure effective integration of treatments. The proven results of Evercare's approach are increased preventive care and early detection of potential problems. All of this can lead to better health outcomes and peace of mind for members and their families at lower cost to the health care system.
