BenefitsCheckUp Colorado is a free, confidential screening that helps seniors with limited or moderate incomes find the programs that are available to help them. Adults 55 years or older, caregivers, clergy and social service workers can all make use of this service, to find programs for themselves or for those whom they care. There also are programs available for those with disabilities.

At St. Anthony Hospitals, we know that your privacy is important and assure you that your information is kept confidential. It takes about 30 minutes to complete the short survey. The result? A program information sheet detailing those programs that for which you may be eligible will be sent to you.

To get started, call 303-899-5582 or toll free, 1-866-550-2752. Be ready with some basic information:

• Birth Date
• Social Security Number
• A list of your prescription drugs and dosages
• Your monthly income
• Any assets you might own
