Friendly Phone Calls Reduce Worry of Care Management for Homebound Seniors

Caregivers of homebound elderly have the overwhelming responsibility of piecing together programs and services that enable a person to remain at home successfully. A major piece of this puzzle is simply the assurance that everything is okay on a daily basis. Despite all the planning, something may go wrong - meals not eaten or a medical alert button not activated. When the caregiver, whether a family member, professional case manager, or guardian, cannot monitor the case daily, the plan begins to fail.
Friendly Phone Calls is a new service that provides personal calls once or twice daily to ascertain the homebound senior is okay. Using an individualized "script", calls are placed at agreed upon times so that the senior is available to answer. This service is designed for cognitively alert individuals who understand this is part of a larger plan to assure successful independence and that he or she must answer to the call. If there is no response after three attempts at 10-minute intervals, a call is placed to a pre-determined contact to alert that there may be a problem. Additionally, when the caller determines there is a physical or emotional need or emergent situation, a contact person is notified.

It's that simple - personal calls to assure the well being of the homebound individual.
